  1. feat(models): support newer GPT token params 

    Map newer GPT and reasoning models to max_completion_tokens while keeping Claude-compatible model translation intact.

Normalize default output token limits in both chat-completions and messages flows so Claude Code can use newer GPT models alongside Claude models.
