We use GitHub environments that require deployment review approvals. We have used them many of times without issues.

But today, for the first time, 2 of many similar workflows would not run after the deployment was reviewed and approved. The problem workflows showed this error at the top of the page each time we clicked Approve and deploy:

There was a problem approving one of the gates.

Details:

1. Background - In the middle of a workflow, we have a gate requiring a reviewer's approval to deploy to that environment.
2. All related GitHub environments have identical configurations.
3. This did NOT affect other workflows for the same GitHub repository.
4. All previous jobs in these problem workflows ran without any issues.
5. This happened several times in a row when trying to approve these workflows again to fix the issue.
6. Triggering a new workflow the same way was successful and did NOT have the same issue.

Workarounds:

1. Trigger a new workflow. Run all previous jobs and approve the deployment like normal.
2. ???

Questions:

1. What are the conditions that cause this error? What are reproduction steps? It seemed inconsistent, but tied to certain workflows.
2. Is this new? I'm unable to find any documentation on this error.
3. Does anyone have more information about the error?
4. Are there any better workarounds?

I saw the same error banner again today - There was a problem approving one of the gates.

However, the workflow actually continued this time. So the error did not seem to block the workflow this time. That is new.
