So I started paying for the AI assistance for a project I am doing as I am on a strict timeline and Claude Sonnet 4 on Github Copilot Agent mode repeatedly adds extra code, or doesn't follow the Copilot-Instructions.md file I have, which explicitly states to offer recommendations before changing any code - Yet it will repeatedly go ahead and make tons of code changes, often code that I haven't asked it to add, which is costing me usage credits, telling it to put it right etc...

This is an extremely frustrating issue, it just goes rogue!

We also see this in our organization, 2 weeks ago Copilot was running great, most of the time the PR created was almost perfect. Now? It ignores most instructions and even the changes it makes, they are not correct, and in my opinion it's useless because it's easier to create the PR myself rather than fix the PR created by Copilot.

I find this a lot with most models, not just the one this report mentions. I am working to confirm this, but it seems likely that the culprit is mismanagement of the context, specifically its length. Try simpler, shorter, more direct instructions, keep things simple, add structure, fragment your specs into smaller files that are loaded on demand (this works well for coding standards in a monorepo), and use nested AGENTS.MD files (the shitty name isn't my fault). Link your specs throughout your project folder hierarchy with the @reference syntax as part of the header (my specs have a 'read this first' and 'read this next' section of links at the top and they're all numbered - 00 - overview, 01 - architecture, 03 - workflow, 04 - tasks). Write your own agents specs and keep the most important instructions in bold near the top, and reference your first spec file in the root of your project. Ask your coding agent to help you shorten these specs and agent files and make them more impactful

I love how the "experts" always resort to saying it's something in the instructions and you need to make it more simple. If we are paying for these premium models, we should get premium use out of them. I want the guy in this post. I have tried several different models. I put all kinds of restrictions and workplace rules and card rails and Claude. Absolutely will not follow any will set of instructions at all. It will routinely tell me that it just makes things up because it's faster than trying to read my instructions. So if we cannot get strict adherence out of these models, how in the heck is anyone getting anything done? I should not have to be a certain level of coder to be able to leverage AI to get what I need done. But that seems to be the answer. Everyone wants to go to is make your instructions more simple or simple task like the whole point of AI is to do the complex things.

Your expectations of what these models can do is too high - a premium model won't fix your woes. You're just doing it wrong. I just use vscode+copilot (free version) and it works absolutely fine. My instructions are followed now and it's very easy to get high-quality complex systems out of it. And yes, you do need to be a senior engineer to get the most out of AI right now. That will change, but right now you have to know what you're doing and use the AI to speed things up. That's all it does - speed things up. Vibe coding is more or less useless (and totally fucking stupid if you ask me).

This isn't an argument forum of matter of opinions, This is a bug report relating to AI (Specifically Claude) adding and making stuff up that is not in the Instruction set, even when the Instruction set is short and concise, and clearly states to not add anything extra, and is costing people extra credits/money than it needs too.

Your level of degree in software engineering is irrelevant, your opinion on "Vibe coding" is irrelevant - We have come to the right place to report a bug/issue, that has been verified by the maintainer, and fixes are in place. Please respect the ticket, and leave opinions out of this.

Bryan, I never used the term vibe coding, and i really dislike being wrapped up in that as i built functional software that provides real value to my clients.
The following behavior is not a "vibe coding" issue, this is a model/model provider issue.

I'm experiencing the exact same issue with user-level instruction files (not just repository-level), which suggests this problem affects the entire custom instruction system in Copilot.

My Setup:

• User-level .instructions.md files in ~/Library/Application Support/Code/User/prompts/
• Files are loaded and readable (agent can quote them when asked)
• Instructions are clear, specific, and critical for workflow reliability
• Model: Claude Sonnet 4.5 (via GitHub Copilot in VS Code)

Specific Violations:

verify-before-suggest.instructions.md: Requires verifying command-line flags and technical details in official documentation BEFORE suggesting them.

Agent repeatedly suggested incorrect gh CLI commands without verification:

• GH_PROMPT_DISABLED=1 (does not exist)
• gh config set prompt disabled (incorrect syntax/command)

The instruction file explicitly states: "Never suggest command-line flags, parameters, or options without verifying they exist" and "Fetch official documentation BEFORE making technical suggestions". Both suggestions were hallucinated and cost time debugging why they didn't work.

Pattern: Agent acknowledges instructions when corrected, apologizes, then violates them again in subsequent responses.

The Kicker:
The agent can:

• ✅ Read instruction files perfectly
• ✅ Quote them verbatim when asked
• ✅ Acknowledge violations after the fact
• ❌ Apply them during response generation

This proves the instructions are loaded but either:

• Stripped/compressed before reaching the model
• Deprioritized below other context
• Not properly included in the actual generation prompt

Impact:
Just like the OP, this costs real money (credits wasted on correcting hallucinated commands) and time (debugging why instructions aren't followed). The verify-before-suggest rule exists specifically to prevent costly hallucinations, yet it's systematically ignored.

Evidence This is Infrastructure, Not User Error:
In discussion #167713, a user switched to Kilo Code with the same Claude model and reported "night and day" difference - completing the same tasks in half the time. They noted: "Context is the most important thing, and for some reason, Copilot cripples the LLM." This proves the problem is Copilot's instruction/context preprocessing layer, not the underlying model's capabilities.

Maintainer suggestions in this thread (file location checks, explicit session prompts, shorter instructions, bolding) are workarounds that shift responsibility to users when the platform should reliably deliver context to the model. My instruction files are already concise, well-structured, and properly located,they shouldn't need "dumbing down" or constant repetition for a premium AI service.

Request:
Can GitHub engineering provide transparency on:

1. How instruction files are processed before reaching the model?
2. Token budget allocated to custom instructions vs other context?
3. Whether compression/summarization occurs?
4. Debug logs showing what context the model actually receives?

Without this visibility, we're blindly guessing why a documented feature doesn't work as advertised.
