Copilot Pro+ billing discrepancy: charged $66 despite only 615/1,500 included premium requests used

I'm seeing a clear inconsistency on my Premium Request Analytics page:

Included premium requests consumed	615.9 of 1,500
Billed premium requests	1,644.90
Billed amount	$66.60

Since my included allowance is far from exhausted, I should not be billed for any overages.

My billing breakdown (Group by: Models):

Questions:

1. Why are overage charges applied when the included pool isn't depleted?
2. Is this related to the separate coding agent SKU introduced in November 2025? If so, how are the 1,500 included requests split across SKUs?
3. The billing UI shows a single counter (615.9 / 1,500) but charges suggest a different allocation model. Is this a known UI issue?

I've opened a support ticket (#4180952) but haven't received a response. Has anyone else experienced this?
