GitHub Copilot ignores instructions and responds implemented

I am seeing strange behavior in github.copilot-chat Version 0.41.1 in VSCode Version: 1.113.0 with model GPT-5.3-Codex.

The instructions were ignored, and Copilot responded "implemented" even though the instructions were not executed.
Anyone else has seen such issues?
