Bug Description
在 Hermes Desktop（v0.15.2, Windows 11）中，当一个会话的对话非常长时：
1. 点击左侧切换到另一个会话，再切回来 — 对话区域停留在中间位置，无法定位到最新消息
2. 左侧 会话大纲 可以跳转到最新问题，但最新问题紧接着显示的不是最新回答，而是一条过去的旧回答
3. 继续向下滚动 — 内容循环出现旧消息，最新回答永远不出现（但后台 ...

paper-access 一揽子改造支持「DOI 寻址」：客户端可以直接 GET /api/papers/10.1103/PhysRevLett.103.150502/markdown 拿到对应论文的 markdown。server
端需要把 DOI 反查成 arxiv_id 后走同一套 handler。
