体感速度の改善（タップ→切替が遅い件）
原因＝(1) 遷移時に何も出ない（ローディング無し）／(2) 遷移ごとに318件を毎回取得。
- 即時ローディング表示：各画面に loading.tsx（スケルトン）を追加。タップ直後にヘッダ／タブはそのままで本文がスケルトン表示→「待たされ感」を解消。
- 共通レイアウトの軽量化：中央FABの「次の医院」算出を 318件読み→limit 1 クエリに（全画面遷移で効く）。 ...
