真正的根因：存储设置缺少单例行 + 写入用的是 UPDATE 而非 upsert
上一个 PR 我把症状归因为「缺 minioOnly 列」，但你反馈开关和预签名 TTL 仍然全部点了没反应。这次完整追了一遍「点击 → mutation → 写库」链路，找到独立的第二个根因：
storageSettings 表里根本没有 id=1 这条单例行。
