v0.139.0 prompt_hooks 升级到 workflow.json 后，引入一个不易察觉的行为：modes. mode .prompt_hooks 中与 top-level 同名的 hook 会覆盖 top-level
hook（不是合并）。这导致用户在 top-level 设置的阶段级 hook（如 execute）在 mode 任务中被静默替换。
