Add "Head of Household"

Head of Household will be the main login for thrive. It is assignable to any account that is created. Remove the dev tag.

New thrive install:
host/client
Head of Household
Name:
username:
password:

this will create an account, the user from Name, link them and assign the user Head of Household.

On the user selection screen the Head of Household's Name will show and an Icon on the avatar will show
